I understand that by tendering this application for nomination I confirm that I am eligible to be a Director under the Australian Organic Constitution.

I am a current “Financial Member” of Australian Organic, per the below definition.

I have not been the subject of any criminal or civil penalty proceedings or other enforcement action by any government agency in which I was found to have engaged in behaviour involving fraud, dishonesty, misrepresentation, concealment of material facts or breach of duty.

I have not been refused membership of, or had my membership suspended or cancelled by, any professional body on the ground that I have engaged in behaviour involving fraud, dishonesty, misrepresentation, concealment of material facts or breach of duty.

I am not aware of any pending or threatened investigation or enquiry by a government agency, professional body, securities exchange or other authority responsible for that could lead to proceedings or action of the type described above

I have not been disqualified from managing corporations under the Corporations Act 2001 (C’wealth) or in any other country or countries.

I, or any company that I was/am a director of, have never been bankrupt or insolvent, taken steps to take the benefit of a law for the relief of bankrupt or insolvent debtors, compounded with one or more creditors, or made an assignment of remuneration for the benefit of one or more creditors.

I consent to Australian Organic Limited and its agents to conduct criminal history and other searches to confirm that I am fit and proper to be a Director.

Upon request, in support of my nomination I will provide a digital passport photograph and a brief written profile of not more than 250 words which is to be distributed to all members with relevant voting papers.
